Valid Electronic Signatures on Forms

Typed names are not considered valid a signature. Signatures completed with a font will not be accepted.

You must sign your name using one of the following methods:

1. Print, sign and scan form
2. Use a stylus, mouse or finger on form
3. Upload an image of your actual signature on form
